Job Description:

We are an Adult Day Health Care in Simi Valley (93065) operating in a completely remodeled and modern building. We are seeking a compassionate and dedicated Adult Day Care Social Worker to join our team.

Part-time position: 8:00-3:00pm Monday-Friday

Job Requirements:

*** Must speak ARMENIAN & ENGLISH fluently

Responsibilities:

Assess clients’ social, emotional, and personal needs.
Develop and update individual care plans.
Help clients access community resources and support services.
Provide emotional support and guidance to clients and families.
Coordinate care with the multidisciplinary team.
Monitor participants’ progress and changing needs.
Maintain accurate client records and documentation.

Qualifications:

PREFERRED:

- Master’s Degree (MS) in Social Work, Psychology, Gerontology, or Counseling

ACCEPTING:

- Bachelors in Social Work (BSW)

Salary:

$28.00 - $30.00 per hour (based on experience)
