Job Description:

The RN/LVN Intake Coordinator is responsible for overseeing all incoming patient referrals and managing the full intake and admission process for home health services. This office-based role ensures that each referral is processed efficiently, accurately, and in compliance with Medicare, state, and agency guidelines. The Intake Coordinator serves as a key liaison between referral sources, physicians, patients, and the clinical team to ensure a smooth and timely transition of care from hospital or community settings to home health services.

This position requires strong clinical judgment, organizational skills, and the ability to manage multiple referrals simultaneously in a fast-paced environment.

Location: Office-based
Schedule: Monday to Friday
Hours: 10:00 AM – 4:00 PM
No weekends or on-call requirements unless otherwise specified
