Job Description:

These positions are for an Adult Day Health Care located in North Hollywood, CA.

The Program Director shall meet one of the following qualifications:

Degree in:

Healthcare Administration
Nursing
Social Work
Psychology
Occupational Therapy
Physical Therapy
Speech-Language Pathology
Dietetics
Gerontology
A related field from an accredited college or university, with a minimum of two years of management, supervisory, or administrative experience

LCSW shall meet all of the following qualifications:

At least one year of experience in social work or a related healthcare setting.
Knowledge of the social, emotional, and behavioral needs of older adults and individuals with chronic medical conditions or disabilities.
Ability to conduct assessments, develop care plans, and work collaboratively as part of an interdisciplinary team.
Experience providing counseling, support, education, and coordination of appropriate community resources and services.
Strong communication, organizational, and documentation skills.
Knowledge of applicable federal, state, and Community-Based Adult Services (CBAS) regulations.
Demonstrates professionalism, compassion, and a commitment to providing quality participant care.

Job Requirements:

The Program Director shall meet all of the following requirements:

A minimum of one year of experience working in an Adult Day Health Care (ADHC/CBAS) program or a similar healthcare setting serving older adults or individuals with chronic medical conditions, physical disabilities, or cognitive impairments.
Knowledge of the physical, mental, cognitive, emotional, and social needs of the participant population served.
Experience leading, supervising, and supporting staff in a healthcare or community-based setting.
Ability to develop, implement, coordinate, and evaluate programs that respond to participants' changing needs.
Ability to oversee daily operations while ensuring services are delivered in accordance with each participant's individualized plan of care.
Ability to work collaboratively with the interdisciplinary team to coordinate services and promote high-quality participant care.
Ability to monitor program effectiveness, identify opportunities for improvement, and make adjustments to better meet participants' needs.
Ability to provide guidance, mentorship, and ongoing support to employees while fostering a positive, respectful, and collaborative work environment.
Strong leadership, communication, organizational, and problem-solving skills.
Ability to build positive working relationships with participants, families, staff, healthcare providers, community partners, and outside agencies.
Demonstrate professionalism, integrity, accountability, compassion, and a commitment to providing safe, high-quality, participant-centered Adult Day Health Care services.
CPR License
Must be able to speak Armenian and English
