Job Description:

Medimax is seeking a motivated and reliable Physical Therapist (PT) to perform Start of Care (SOC) and Physical Therapy Evaluations for home health patients throughout the community.

Responsibilities

Complete Start of Care PT evaluations in patients’ homes
Develop individualized treatment plans
Assess mobility, safety, functional status, and equipment needs
Communicate with home health agencies and physicians as needed
Complete documentation accurately and on time

Job Requirements:

Active California Physical Therapist License
Valid driver’s license and reliable transportation
Home health experience preferred
Strong clinical and communication skills
Ability to work independently
