As a Hospice Registered Nurse, you will be responsible for providing compassionate, patient-centered care to individuals with terminal illnesses in their homes or other care settings. Your role will involve assessing patient needs, managing pain and symptoms, educating families, and coordinating care with a multidisciplinary team.

Key Responsibilities:
Provide skilled nursing care to hospice patients, including symptom management, pain control, and comfort measures
Assess patient conditions, develop care plans, and monitor progress
Communicate effectively with patients, families, and the care team to ensure optimal care
Administer medications and treatments as prescribed by the physician
Offer emotional support to patients and families during end-of-life care
Ensure adherence to hospice policies and regulations
Document patient care and progress accurately and timely


As a Hospice LVN, you will provide direct nursing care to hospice patients, assist with symptom management, and offer comfort and emotional support to patients and their families. You will work under the supervision of RNs and physicians, delivering compassionate care in a variety of settings, including patients’ homes.

Key Responsibilities:
Provide basic nursing care to hospice patients, including administering medications, treatments, and personal care
Monitor patient symptoms, including pain, and report changes to the RN or physician
Assist in developing and implementing care plans, ensuring patient comfort and dignity
Offer emotional support to patients and families
Collaborate with RNs, physicians, and the interdisciplinary team to ensure holistic care
Maintain accurate patient records and ensure proper documentation
Comply with all hospice policies and procedures
