- Conduct comprehensive assessments of patients' social, emotional, and environmental needs
- Develop individualized care plans in collaboration with the healthcare team
- Provide counseling and support to patients and their families regarding coping strategies, treatment options, and available resources
- Assist with discharge planning and coordination of post-hospital care
- Collaborate with interdisciplinary teams to ensure continuity of care
- Conduct group therapy sessions to address specific patient needs

- Assist with medical record documentation for reimbursement purposes
- Provide education on dementia care, addiction counseling, and other relevant topics
- Offer crisis intervention services as needed
Qualifications:
- Master's degree in Social Work from an accredited program

- Experience in mental health counseling, crisis management, and medical social work preferred
- Knowledge of applied behavior analysis principles is a plus
- Strong interpersonal skills and the ability to establish rapport with diverse populations
- Excellent communication skills, both verbal and written
- Ability to work collaboratively within a multidisciplinary team
- Proficiency in medical record documentation .
