Job Description:

As a PT or OT with Comfort at Home, you will:

-Perform comprehensive patient evaluations in the home setting
-Develop individualized plans of care based on each patient's needs and physician orders
-Provide skilled physical or occupational therapy treatments
-Educate patients, family members, and caregivers on treatment plans, exercises, safety, and home programs
-Assess patient progress and modify treatment plans as appropriate
-Complete accurate and timely clinical documentation
-Communicate and coordinate care with physicians, nurses, therapists, and other members of the interdisciplinary team
-Promote patient independence, mobility, safety, and overall quality of life
-Follow all applicable home health, Medicare, state, and agency requirements

Job Requirements:

-Current and active California Physical Therapist (PT) or Occupational Therapist (OT) license
-Graduate of an accredited PT or OT program
-Valid driver's license and reliable transportation
-Ability and willingness to travel to patients' homes within your selected coverage area
-Strong communication and documentation skills
-Professional, dependable, and patient-focused
Coverage Areas
We are currently hiring clinicians who can cover one or more of the following areas:
LANCASTER • PALMDALE • ANTELOPE VALLEY • SAN FERNANDO VALLEY • LOS ANGELES COUNTY • KERN COUNTY
You do not need to cover every service area. Applicants may indicate their preferred cities, regions, and travel radius.
