Receive and process new patient referrals in a timely and accurate manner.
Review referral documentation for completeness and determine whether all required information has been received.
Enter and maintain patient and referral information accurately in Synergy.
Verify patient demographics, contact information, diagnosis, insurance coverage, and other required admission information.
Verify insurance eligibility and benefits and obtain authorization when required.
Review physician orders, history and physicals, hospital documentation, medication information, and other referral records.
Identify missing documentation and promptly follow up with hospitals, physicians, case managers, discharge planners, patients, families, and referral sources.
Coordinate referral acceptance with clinical and administrative staff.
Coordinate Start of Care (SOC) and ensure referrals are appropriately assigned and scheduled.
Communicate with nurses, therapists, physicians, case managers, marketers, and other members of the interdisciplinary team.
Maintain communication with referral sources regarding referral status, acceptance, delays, missing documentation, and start-of-care arrangements.
Ensure all required documentation is obtained before admission whenever applicable.
Confirm physician information and obtain signed orders and supporting documentation as needed.
Track pending referrals and follow up consistently until the referral is admitted, declined, or otherwise resolved.
Maintain accurate referral logs and intake records.
Update patient status and documentation in Synergy throughout the intake process.
Assist with coordinating staffing for new admissions and communicate scheduling needs to the appropriate departments.
Ensure intake documentation complies with company policies and applicable regulatory requirements.
Protect patient confidentiality and maintain compliance with HIPAA requirements.
Communicate professionally and compassionately with patients and family members throughout the intake process.
Answer incoming intake calls and respond promptly to referral-related emails, faxes, and electronic communications.
Prioritize urgent referrals and communicate time-sensitive issues to management and clinical staff.
Identify barriers that may delay admission and escalate issues appropriately.
Maintain organized and complete electronic patient records.
Collaborate with management, clinical staff, billing, authorization, scheduling, and other departments to ensure a smooth admission process.
Perform follow-up calls as necessary and provide excellent customer service to referral sources.
Assist with additional intake, administrative, and coordination duties as assigned
