Job Description:

Seeking a detail-oriented and organized Intake Coordinator to manage patient referrals and admissions for our home health agency. The Intake Coordinator will work closely with hospitals, physicians, case managers, and clinical staff to ensure a smooth and timely patient intake process.

Job Type: Full-Time | Monday–Friday | 10:00 AM – 5:00 PM

Job Requirements:

Key Responsibilities
Receive and process patient referrals and admissions for home health services
Verify insurance eligibility and benefits (Medicare, Medi-Cal, private insurance)
Coordinate start-of-care visits with nurses, therapists, and other clinical staff
Communicate with physicians, hospitals, and referral sources to obtain required documentation
Maintain accurate patient records in the electronic medical record (EMR Synergy) system

Qualifications

High school diploma or equivalent required; healthcare-related education preferred

1–2 years of experience in healthcare intake, medical office, or home health preferred

Strong communication, organizational, and multitasking skills

Fluent in English and Armenian (required)

Knowledge of insurance verification and patient admissions processes

Preferred Skills
Experience in home health or hospice intake
Familiarity with home health EMR systems (Healthcare Synergy)
Proficiency with Microsoft Office and electronic referral systems
Additional languages preferred: Farsi, Russian, or Spanish
