The Intake Coordinator is responsible for verifying eligibility, coordinating assessments, and facilitating communication between the interdisciplinary team and external stakeholders
**Patient Intake Process:**
Receive and process referrals from physicians, hospitals, and other healthcare providers
Verify patient eligibility for hospice care based on medical criteria and insurance coverage
Collect and document all necessary patient information, including demographic details, medical history, and referral source data
**Coordination of Care:**
Schedule initial assessments and coordinate with the hospice interdisciplinary team (IDT), including nurses, physicians, and social workers
Ensure timely communication with patients and families to explain the hospice admission process and answer initial questions
Liaise with external providers and facilities to ensure continuity of care during the transition to hospice
**Documentation and Compliance:**
Maintain accurate and detailed records of all referrals and admissions in compliance with HIPAA regulations and organizational policies
Ensure completion of necessary forms, including consent, authorizations, and insurance verification
Monitor and update electronic medical records (EMR) with intake and admission details
**Customer Service:**
Provide compassionate support to patients, families, and caregivers during the intake process
