Hospice Clinical Supervisor Needed - Van Nuys, CA

Supervise and mentor clinical staff, guiding best practices in patient care.
Review available patient information related to the case, including home visits, to determine hospice/home care needs. Assign appropriate hospice personnel to the case as needed. Conferences with MEDICAL DIRECTOR regarding any questions about an individual’s eligibility for services.
Review the patient’s medical diagnosis, prognosis, medications, procedures, and clinical course.
Oversee medical documentation and records management to ensure accuracy and completeness.
Collaborate with healthcare professionals to develop and implement patient care plans.
Conduct regular assessments of patient care practices, identifying areas for improvement.
Provide ongoing training and development for clinical staff.
Facilitate communication between patients, families, and healthcare providers to ensure optimal care.
Monitor clinical outcomes and report findings to management for continuous quality improvement.
