Home Health QA Needed - La Crescenta, CA

Key Responsibilities

Prepare, review, and coordinate OASIS documentation with assessing clinicians.

Review OASIS assessments for accuracy, completeness, and compliance with applicable Medicare guidelines.

Prepare and update Plans of Care (CMS-485), physician orders, and recertification documentation.

Review nursing and therapy documentation for accuracy, completeness, and compliance.

Identify documentation deficiencies, discrepancies, and missing information.

Follow up with clinicians to obtain corrections, missing documentation, and required updates.

Coordinate physician orders and follow up on outstanding physician signatures.

Monitor patient charts to ensure all required documentation is complete, accurate, and ready for billing.

Maintain organized patient records in accordance with Medicare requirements and Home Health Conditions of Participation.

Communicate effectively with clinical staff, physicians, and office personnel to resolve documentation issues.

Previous Home Health QA experience is required.
