We are seeking a detail-oriented and experienced Home Health Quality Assurance (QA) Reviewer to perform weekly reviews of discharged patient charts. This is a limited part-time position designed for someone who can dedicate a few hours once per week to come to the office and ensure completed charts are accurate, complete, compliant, and ready for billing and audit purposes.

Primary Responsibilities:

Review discharged patient charts on a weekly basis.

Conduct a final QA review of clinical documentation prior to chart closure.

Verify that all required nursing, therapy, and other discipline visit notes are present and complete.

Review SOC, ROC, Recertification, Transfer, and Discharge documentation when applicable.

Ensure OASIS assessments and discharge documentation are completed appropriately.

Verify physician orders, Plans of Care/485s, verbal orders, and supplemental orders are present and signed as required.

Confirm visit frequencies and disciplines provided are consistent with physician orders and the Plan of Care.

Identify missing visits, missing documentation, unsigned notes/orders, date discrepancies, and other chart deficiencies.

Verify required discharge documentation, including discharge summaries and applicable notices, is completed.

Review documentation for consistency between clinical notes, orders, frequencies, and the patient's course of care.

Ensure documentation supports medical necessity and skilled services provided.

Confirm charts meet Medicare, CMS, agency, and applicable accreditation requirements.

Create a list of deficiencies or outstanding items requiring correction or follow-up.

Communicate identified issues to the Administrator, Director of Nursing, clinicians, or appropriate office staff.

Recheck corrected documentation when necessary and confirm the chart is complete prior to final closure.

Maintain patient confidentiality and comply with HIPAA requirements.
