Requirements:
Minimum 3 years of home health intake experience
Synergy system experience required
Knowledge of Medicare home health requirements
Experience with insurance verification and authorization
Strong communication, organization, and follow-up skills
Ability to manage multiple referrals and admissions efficiently
Responsibilities:
Process and coordinate new patient referrals
Verify Medicare and insurance eligibility
Review referral documents and physician orders
Coordinate Start of Care (SOC)
Communicate with physicians, hospitals, referral sources, patients, and clinical staff
Enter and maintain accurate patient information in Synergy
Follow up on missing documentation and pending admissions
Ensure timely and compliant intake processing

Experience: Home Health Intake – 3 years minimum (Required)
