Job Description:

Home health agency located in Canoga Park is looking for an experienced Home Health Intake Coordinator. The ideal candidate must have prior home health experience, be familiar with Kinnser/WellSky software, and be fluent in English. This is a full-time position, Monday through Friday, from 9:00 AM to 5:30 PM.

Responsibilities include receiving and processing referrals, verifying patient information, checking insurance eligibility, coordinating with physicians, patients, and clinical staff, entering patient data into Kinnser/WellSky, and assisting with intake documentation to ensure timely start of care.
