Job Description:

We are currently seeking an experienced Home Health Intake Coordinator who is proficient in Synergy EMR.
Current census is 6 patients

Responsibilities include:
• Managing and processing new patient referrals
• Verifying insurance and eligibility
• Coordinating with physicians, hospitals, and case managers
• Entering and maintaining accurate patient information in Synergy
• Ensuring timely start of care and proper documentation

Job Requirements:

Requirements:
• Minimum 2 years of home health experience
• Hands-on experience with Synergy EMR (required)
• Fluent in English
• Strong communication and organizational skills
• Ability to work independently and efficiently
