Our home health is looking for EXPERIENCED Intake Coordinator

•Scheduling all the steps starting from Admission to Discharge of the patient (KINNSER)
•Screen clients for eligibility for direct and support services, obtaining orders and referring clients to needed services, such as physical therapy, occupational therapy, speech therapy, social worker, mental health etc.
•Scheduling nurses
•Maintain all patients documentation
•Track client information, schedules, files and forms in a confidential manner
•Communicating with doctor's offices and hospitals
•Communicating with patients for wellness follow up checks
•Other assignments.
