We are seeking a detail-oriented and organized Intake Coordinator to join our team. This role is responsible for managing incoming referrals, verifying patient eligibility, coordinating start of care, and ensuring a smooth intake process and continued care for patients

Key Responsibilities:

Receive and process new patient referrals from hospitals, physicians, facilities, and
community partners.

Review referral documents for completeness, including demographics, insurance,
diagnoses, medication lists, discharge paperwork, physician orders, and face-to-face
documentation.

Verify eligibility and confirm that the patient meets the agency’s admission
criteria.

Confirm the referring or certifying physician is eligible to sign home health orders
(PECOS enrolled).

Follow up for missing documents or orders.

Enter patient and referral information accurately into the EMR.

Assign nurses, therapists, and home health aides based on location, availability, and
patient needs.

Monitor and document missed, refused, delayed, and rescheduled visits.

Prepare, send, track, and follow up on plans of care, orders, therapy orders,
recertification orders, and discharge orders.

Maintain an outstanding-orders log and follow up with physician offices for signatures.

Review charts for missing documentation, signatures, dates, and required forms.

Ensure signed orders and supporting records are uploaded into the correct patient chart.

Maintain organized physical patient records.

Answer and direct telephone calls, emails, faxes, and patient inquiries.

Assist with employee onboarding, personnel files, credential tracking, and annual
requirements.

Follow Medicare Conditions of Participation, HIPAA requirements, agency policies, and
accreditation standards.

Monitor charts for required admission, recertification, transfer, and discharge
documentation.

Maintain confidentiality of patient and employee information.

Help ensure documentation and signed orders are available before claims are
submitted.

Help prepare agency admission packets.
