Job Description:

We are looking for an EXPERIENCED Intake / Care Coordinator. Responsibilities include:

- Scheduling all the steps starting from Admission to Discharge of the patient (Kinnser/WellSky)
- Contacting patients and providers to coordinate care and obtain documentation.
- Checking eligibility for direct and support services, obtaining orders, and referring patients to needed services such as Skilled Nursing, Physical Therapy, Occupational Therapy, Speech Therapy, Social Worker services, and Home Health Aid.
- Scheduling nurses, therapists, and aids
- Maintaining patient documentation and tracking their information and schedules in a confidential manner.
- Communicating with providers (doctors' offices and hospitals)
- Communicating with patients for wellness follow-up checks
- Other assignments

Job Requirements:

Must have experience working in Kinnser/Wellsky
Fluent English required
