Job Description:

We are looking for a skilled and compassionate Home Health Care Coordinator / Case Manager to join our team. This role focuses on coordinating personalized care for patients in their homes, ensuring they receive the services and support they need to achieve optimal health outcomes.

- Assess patients' needs, develop individualized care plans, and evaluate progress.
- Coordinate with an interdisciplinary team, including nurses, therapists, and social workers, to deliver high-quality home health services.
- Act as a liaison between patients, families, and healthcare providers to ensure seamless communication and care transitions.
- Educate patients and families on care plans, medications, and available resources.
- Monitor patient outcomes, document progress, and adjust care plans as needed.
- Advocate for patient needs, ensuring they receive appropriate care and services.

Job Requirements:

- Experience in home health, care coordination, case management, or related healthcare fields preferred.
- Strong organizational and time-management skills.
- Excellent communication and interpersonal abilities.
- Knowledge of Home Health Care regulations and best practices.
- Fluent English is required
