Home Health Agency Quality Assurance Coordinator (QA) Needed - Glendale, CA

We are seeking an experienced and detail-oriented Quality Assurance Coordinator to join our growing home health agency. In this role, you will serve as a key driver of clinical excellence, ensuring that our patient care practices, documentation standards, and operational processes remain fully compliant with state and federal regulations, accreditation standards, and agency policies. You will work closely with clinical leadership, field staff, and administration to foster a culture of continuous quality improvement.
• Conduct routine clinical record audits, including OASIS accuracy reviews, care plan evaluations, and visit note compliance.
• Monitor and evaluate adherence to state and federal home health regulations (Medicare Conditions of Participation, Medicaid requirements).
• Prepare agency for and participate in accreditation surveys (ACHC) coordinating mock surveys and corrective action plans.
• Track, analyze, and report on quality indicators, patient outcomes, and adverse events; present findings to leadership.
• Identify deficiencies and trends in clinical documentation and collaborate with supervisors to implement corrective actions.
• Develop and maintain QA policies, procedures, and audit tools to reflect current regulatory standards.
• Support the QAPI (Quality Assurance and Performance Improvement) program, including committee participation and data-driven improvement initiatives.
• Serve as a resource and educator for clinical staff on compliance requirements, documentation best practices, and regulatory updates.
• Coordinate responses to state surveys, complaint investigations, and audit findings from payers or accreditation bodies.
• Maintain current knowledge of CMS guidelines, OASIS guidance, and accreditation standards to keep agency practices up to date.
