We are seeking an experienced Intake Coordinator to support the daily operations of our Home Health agency. This role is essential to ensuring efficient coordination of patient services, effective support of clinical staff, and accurate documentation and communication across the organization.

Primary Responsibilities:
- Contact patients and/or caregivers to confirm demographic, insurance, and referral information
- Coordinate and communicate with nurses, PT/OT, and other clinicals regarding new admissions and ongoing services
- Schedule start-of-care, recertification, and routine visits in accordance with physician orders and agency timelines
- Follow up on referrals to ensure timely initiation of services
- Receive, review, and process referral documentation for completeness and accuracy
- Enter and update patient information in the electronic medical record system
- Communicate effectively with physicians’ offices, hospitals, and referral sources as needed
- Perform general administrative and documentation tasks to support intake and clinical operations
- Maintain confidentiality and comply with HIPAA and agency policies
