Job Description:

We are seeking a compassionate and dedicated Field LVN to provide high-quality, patient-centered care in the home health setting. As a Field LVN, you will work closely with patients in their homes, ensuring they receive the necessary medical attention, monitoring their progress, and supporting their overall health and well-being. You will be part of a dynamic healthcare team, delivering skilled nursing services, education, and assistance to patients in the comfort of their homes.

Job Requirements:

Must be a licensed LVN
Previous experience in home health
Strong clinical assessment skills and the ability to recognize and act on changes in patient condition.
Excellent communication skills to effectively educate patients, and families, and collaborate with the healthcare team.
Ability to work independently and manage time efficiently.
Compassionate and patient-focused with a strong desire to improve patient outcomes.
Reliable transportation and ability to travel to patient homes as required.
