Job Description:

Home Health is seeking an EXPERIENCED Intake Coordinator to join our growing team. We currently serve over 95 active patients and receive referrals from hospitals, skilled nursing facilities, board and care facilities, physicians, and community referral sources.

The Intake Coordinator will be responsible for managing the intake process from referral to admission, coordinating patient services, staffing and scheduling clinicians, verifying insurance eligibility, coordinating physician visits for SOC/ROC/Recertifications, and maintaining communication with referral sources and clinical staff. This position requires excellent organizational skills, strong communication abilities, attention to detail, and the ability to work in a fast-paced environment.

Schedule: Monday–Friday, 9:00 AM–5:00 PM
Compensation: Starting at $30.00/hour (W-4 Employee)
Benefits: Paid Holidays, 30-Minute Paid Lunch

Job Requirements:

- Minimum 2 years of home health intake experience
- Required experience with Synergy EMR
- Strong understanding of Medicare home health regulations and processes
- Proficient in English (reading, writing, and verbal communication)
- Experience performing insurance eligibility and benefits verification
- Experience coordinating staffing and clinician scheduling
- Ability to schedule and coordinate physician visits for SOC, ROC, and Recertification patients
- Excellent email communication skills with prompt and professional response times
- Strong organizational and multitasking abilities
- Reliable, dependable, and able to work independently
- Proficient with Microsoft Office, Outlook, and general computer applications
- Professional demeanor when communicating with referral sources, patients, physicians, and agency staff
- Must be detail-oriented and able to manage a high-volume patient census efficiently
- Applicants must be available Monday through Friday, 9:00 AM – 5:00 PM and committed to maintaining consistent attendance and reliability.
