Job Description:

We are seeking a detail-oriented and organized Case Coordinator to join our home health team.

Key Responsibilities:
Coordinate and schedule patient visits for skilled nursing, therapy, and home health aide services
Serve as a liaison between patients, clinicians, physicians, and referral sources
Create all orders in DSL (medication, IV, labs etc.)
Manage and track orders sent to pcp, ensuring timely follow-up and signatures
Maintain accurate and up-to-date patient records in DSL
Monitor visit frequencies and ensure adherence to care plans
Assist with intake processes, including insurance verification and authorization tracking
Communicate changes in patient condition or scheduling needs to appropriate staff
Ensure compliance with agency policies, state, and federal regulations
Support clinical team with administrative tasks as needed

Job Requirements:

MUST SPEAK FLUENT ENGLISH AND HAVE HH WORK EXPERIENCE. Hours are M-F 10-4pm

Previous experience in home health is required
Knowledge of home health regulations and processes (OASIS, Medicare guidelines)
Strong organizational and time management skills
Excellent verbal and written communication skills
Ability to multitask and work in a fast-paced environment
Proficient in DSL

Compensation based on experience
