Job Description:

Lead Care Manager who is fluent in Armenian and English. This person will help clients manage their health, appointments, services, and daily needs.

This is an office-based position in Burbank. The Lead Care Manager must also be able to complete face-to-face visits with clients when needed or when requested by the client.

Job Duties
Call clients and stay in regular contact with them.

Complete client assessments to learn about health, housing, food, transportation, mental health, safety, and other needs.

Create and update care plans based on each client's needs and goals.

Help clients understand their care plan and the steps they need to take.

Help schedule doctor, specialist, therapy, and other appointments.

Follow up after appointments to make sure the client received the needed care.

Speak with doctors, hospitals, pharmacies, social workers, and other service providers.

Help clients with transportation, food, housing, benefits, and community resources.

Send referrals and follow up to make sure services were received.

Help clients after hospital or emergency room visits.

Help with follow-up appointments, medications, and other services after discharge.

Keep clear and complete notes after calls, visits, referrals, and other work.

Track client goals, open tasks, referrals, and follow-ups.

Complete face-to-face visits with clients when needed or when requested.

Report urgent health, safety, or other serious concerns to a supervisor.

Keep all client information private.

Attend team meetings, trainings, and case reviews.

Complete other care management and office duties as needed.

Job Requirements:

Fluent Armenian is required because this position regularly works with Armenian-speaking clients.

Must be able to speak, read, and write English at the level needed to perform the job and complete clear work documentation.

Must be able to complete a full job interview in English.

Must be able to write clear work notes in English.

Must have good communication and people skills.

Must be organized and able to manage many tasks and follow-ups.

Must be comfortable using a computer and entering notes into a system.

Must be dependable and able to work with little supervision.

Must be able to work in the Burbank office.

Valid driver's license is required.

Reliable transportation is required.

Must be able and willing to travel locally for client visits when needed.

Must be legally authorized to work in the United States.

Employment eligibility verification is required.

Preferred Experience

Experience in healthcare, care management, case management, social services, community work, or a similar role is preferred.
