Job Functions/Responsibilities:

• Coordinates home care services for patients assists with the establishment of the plan of care in coordination with the physician and the DON.
• Maintains comprehensive working knowledge in the field and shares information with appropriate organization personnel.
• Works with Clinical Supervisors/Nursing Supervisors and staff clinicians to gather, collate, and report patient care summary information.
• MUST have experience checking and understanding OASIS-E to clarify proper data entry in timely matter.
• MUST have experience reading and understanding new physician orders and generating new home health orders in a timely manner to be sent to the physicians.
• Maintains comprehensive working knowledge of contractual relationships and ensures that patients are admitted and services are delivered in accordance with contract provisions.
• Understanding and clarifying SOC plan of care (485) and discharge plans including discharge barriers and patient/family education.
• Discusses w/ patient, family/significant others about the plan of care.
• Monitors the patient's progress based on their care plan.
• One-to-Two-year minimum clinical case management experience.
• Currently licensed as a RN/LVN in the State of CA.
• Responsible for the coordination of home health services, appropriately reporting the identified needs for home health interdisciplinary group members, including home health aide, OT, PT, MSW, ST, Dietician or Clinical Supervisor
• Experience with an EMR charting system.
